If the pet seems unwell, call the vet first, then the owner. Made with goodpetday.me; nothing on this sheet leaves this device.

How to use it

Fill in whatever your sitter needs and leave the rest blank: blank fields print as empty lines you can write on by hand. The form saves itself in this browser as you type, so you can come back and update it before each trip. When it is ready, print it or download a plain-text copy to send in a message.

Type medications exactly as they appear on the vet's label or prescription, including when to give them and anything to watch for. The sheet prints your words as they are and tells the sitter to call you or the vet if anything is unclear; it never checks or works out doses. Add an out-of-hours vet as well as your usual one, since emergencies rarely keep office hours.

Questions

Where is my information kept?

Only in this browser's local storage, on this device. Nothing is uploaded or sent anywhere. "Clear form" removes the saved copy, and so does clearing this site's data in your browser. On a shared computer, clear the form when you are done.

What is worth including?

Exact amounts and times for food, every medication, your vet and an out-of-hours vet, a backup contact nearby, and what is normal for your pet, such as hiding during storms or never eating breakfast. Those details help a sitter tell an off day from a real problem.

Can I make sheets for two pets?

The page keeps one sheet at a time. Fill in the first pet, print or download it, then change the details for the next pet and print again.
